An adult case of empyema necessitans caused by <i>Porphyromonas gingivalis</i>

چکیده
胸壁穿通性膿胸（Empyema necessitans）は膿胸腔内に貯留した内容物が胸壁を貫き，胸腔外の周辺組織に膿瘍を形成する病態である．原因は結核菌や放線菌の慢性感染が多く報告されている．今回，Porphyromonas gingivalisによる胸壁穿通性膿胸を経験した．症例は72歳の女性．左側腹部の皮下腫瘤に気づき前医を受診した．CT，MRIで左膿胸を認め，膿胸に連続する左側腹部皮下の液体貯留を認めたため胸壁穿通性膿胸と診断した．入院後早期に胸腔鏡下膿胸腔掻爬ドレナージ術，皮下膿瘍ドレナージ術を施行した．P. gingivalisは歯周病の原因菌で，蛋白質の分解産物を栄養源としているため強力な蛋白質分解酵素を有しており，重篤な膿瘍形成をきたす傾向が強い．今回，手術により良好な結果が得られたため，文献的考察を加え報告する．
